When the student completes their driver education class, they will receive a certificate of completion.
This certificate is needed when the student goes to the Department of Transportation to get their driver license.
In order to get a duplicate certificate of completion, we must have the student's name and address. Please give us the date the student completed the class and in what city/location they took the class.
There is a $25 fee and the student will be required to drive with the Department of Transportation if a certificate has to be reissued.
Once we receive the information and payment a new certificate of completion will be issued. If a personal check is used a form of payment, we will hold the certificate for three weeks for the check to clear the bank.- Online Driver Education Registration Form{printable form}
